Edição 224 – Avaliação dos Níveis de Óxido Nítrico em Pacientes Submetidos à Cirurgia Cardíaca com Circulação Extracorpórea sob Normotermia ou Hipotermia

Resumo

A circulação extracorpórea (CEC) em cirurgia cardíaca pode levar ao desenvolvimento de uma resposta inflamatória sistêmica (SRIS), por meio da liberação de mediadores inflamatórios como o óxido nítrico (NO). Objetivo: investigar a produção sistêmica de NO em pacientes submetidos à cirurgia cardíaca com CEC, sob normotermia (N) ou hipotermia (H). Amostras de sangue foram coletadas em diferentes tempos após início da cirurgia e o soro armazenado a -20ºC. Metabólitos de NO foram analisados por meio da reação de Griess. Resultados: não foram detectadas diferenças significativas na produção de NO entre os tempos avaliados no grupo N, porém, no grupo H, foi observada redução nos níveis sistêmicos em T1 e T2, comparado a T0. Houve recuperação nos níveis de NO em T3, quando estes não foram significativamente diferentes de T0, ocorrendo posterior redução em T4. Nos grupos comparados entre si, para verificar possíveis diferenças entre os tempos, detectou-se aumento nas concentrações séricas de NO em T0 no grupo H, comparado ao grupo N. A redução da produção sistêmica de NO detectada no grupo H demonstra o papel da CEC como agente de injúria, independentemente das condições de temperatura do organismo, podendo ser prejudicial, considerando que o NO é importante para evitar disfunção pulmonar.

Summary

Cardiopulmonary bypass (CPB) in cardiac surgery can lead to the development of a systemic inflammatory response (SIRS), through the release of inflammatory mediators such as nitric oxide (NO). Objective: to investigate the systemic production in patients undergoing cardiac surgery with CPB under normothermia (N) or hypothermia (H). Blood samples were collected at different times after the beginning of surgery and the serum stored at -20ºC. NO metabolites were analyzed by the Griess reaction. Results: Significant differences were detected in NO production between the times evaluated in the N group, however, the H group was observed reduction in systemic levels in T1 and T2 compared to T0. There was reduction in NO levels at T3 when these were not significantly different from T0 occurring subsequent reduction in T4. In groups compared, to check for possible differences between times, detected an increase in serum concentrations of NO at T0 in H group compared to the group N. The reduction of systemic production of NO detected in the H group demonstrates the role of CEC as injury agent, regardless of the temperature of the organism may be harmful, given that NO is important to prevent pulmonary dysfunction.
